Athletic Physical Forms

Students must submit a completed IHSAA Physical Exam form and several additional forms annually to be eligible to begin practicing for a school sport.

For the 2020-21 school year, athletes with a valid IHSAA Physical Exam from last school year will have that exam extended for this school year, also.  However, you will need to complete the 2020-21 IHSAA Health History Questionnaire and Consent Form (accessed at the link at the left).  You also need to update all of your Final Forms forms.

Any students who are new to athletics this school year and don't have a valid physical form from last year on file will need an IHSAA Physical Exam (link to form at left). This exam must be on file before beginning any summer workouts, which begin July 6th. 

Here are some guidelines to follow:

1. The current year IHSAA Physical Form must always be completed on hard copy.  It can never be completed online.

2. A program, called Final Forms, is in effect for all Delta sports.  This program enables all forms (other than the actual IHSAA Physical) to be completed online by parents and students.
